Acute Myocardial Infarction Caused by High Voltage Electrical Injury

Abstract
Electrical burns from high voltage current cause multisystem injuries with significant morbidity and mortality. Myocardial damage by high voltage current has often been reported in patients with electrical burn, but actual myocardial infarction
is
uncommon. Clinical presentation of myocardial infarction by electrical damage is variable and easily missed.
We experienced a 39-year-old prevously healthy man with acute myocardial infarction caused by high voltage electrical injury. He sustained electrical injury (25,000 V, alternating current) while working on the railroad. On admission, MB fraction
of
creatine phosphokinas was increased and electrocardiogram showed marked elevation of ST segment in lead II, III, aVF and V5-6 suggesting of inferolateral myocardial injury. 2-dimensional echocardiogram showed moderate hypokinesis of inferior wall
of
left ventricle. 3 month after injury, follow-up echocardiogram revealed no significant interval change.
